Dear Vikings, 

All students who are compliant with the SSU vaccine mandate and have uploaded proof of their vaccination status or received an approved exemption by Monday, August 23 at 5 pm will be eligible to win prizes. Please visit this link to learn more about the SSU vaccine mandate and how to access the vaccine.

To enter this contest, students must have already or will upload proof of their vaccination status into the student health portal, which is available at this link, by Monday, August 23 at 5 pm. All current Salem State students* are eligible to enter, and winners will be randomly selected for these taxable prizes: 

  • One student will win $2,000 in credit towards their Salem State account. That’s right, your bill will be reduced by $2,000! 
  • The second prize will be awarded to four students: Follett book scholarships of $500
  • The third prize will also be awarded to four students: ClipperCash credits of $250 each 



How to Enter

All students who are compliant with the Salem State vaccine mandate are eligible to win and will be entered into this contest. This includes all students who are fully vaccinated as well as students who have received exemptions. If you have not yet submitted your information, please upload your proof of vaccination or submit an exemption to the student health portal by Monday, August 23 at 5 pm. For information about how to log in to the student health portal, please visit this link.

 

*Contest Rules

Each student is only eligible to win one prize. Winners will be randomly selected by Salem State’s counseling and health services department and prizes will be awarded to the appropriate student account; no cash prizes will be given. Full-time employees of Salem State University are not eligible. Winnings will be subject to state and federal tax laws. 

 

Eligibility

You are eligible for the drawings only if you meet all the following requirements at the time of entry and the time of any potential prize award:

  1. You are a current Salem State student registered for the fall semester.

  2. You are at least 17 years of age.

  3. You have received the full required regimen of vaccine doses as specified by the FDA:

    • If you are receiving the Pfizer or Moderna vaccine, you have received both doses of the vaccine

    • If you are receiving the Johnson & Johnson vaccine, you have received the one required dose of the vaccine

    • Or you have been approved for a medical or religious exemption. 

  4. You are not an employee or officer of Salem State University or an immediate family member (spouse, domestic partner, child, parent, parent-in-law, or sibling) who resides in the same household as such an officer or employee.

  5. You are not an officer, contractor, or employee who assisted in the development or administration of this contest, or an immediate family member (spouse, domestic partner, child, parent, parent-in-law, or sibling) who resides in the same household as such an officer, contractor, or employee.

 

Drawings

In order to ensure your medical privacy, the winners will be randomly selected by the counseling and health services department from among all eligible entries received before the deadline of Monday, August 23 at 5 pm. Salem State will use a random number process to conduct the selection. Winners will be contacted by Friday, August 27. 

 

Verification

Salem State will review and attempt to confirm potential winner information and eligibility prior to declaring an official winner for each prize. Verification of all information and eligibility criteria must be established to the sole satisfaction of Salem State before a potential winner is named an official winner or entitled to claim any prize.

 

Taxes

Official prize winners are responsible for all taxes, including income taxes. Official Winners 18 years of age or older will receive a W-2G form from Salem State reflecting the value of their prize. All amounts or values credited to an official winner will be included in the taxable income that is reported on the official winner’s W-2G form and the official winner will be taxed on those amounts. The minimum required federal and state taxes will be withheld from all prizes awarded to official winners 18 years of age and older.

 

General

Salem State University is not responsible for late or incomplete or misdirected participant entries, or for incorrect, inaccurate or incomplete information associated with such entries, even if caused by failures in the student health portal, or by any of the equipment or programming associated with or utilized in this promotion.

All materials submitted by participants in connection with the promotion become the property of Salem State and will not be returned. Prizes are non-transferable to any entity or other person, including relatives or friends.

Salem State University, in its sole discretion, reserves the right to select or change the prize(s) to be awarded as part of this promotion or end the promotion prior to all prize winners being drawn.

These official terms and conditions are subject to review and may be amended or revised at any time at the sole discretion of Salem State University.
